About the event

Having a health care directive is one way of making your wishes known. But, how do we talk about what is important to us? What do we want our loved ones to know? Join Ann Ellison, Advance Care Planning facilitator from MHealth Fairview, in a meaningful discussion about the health care directive and how to communicate your goals, values and wishes. Obtain some tools and resources that will get you thinking, writing, and talking about what is most important to you.

Session will be from 10:30am-12noon with a soup lunch at 12 noon.
